Karachi is Pakistan's largest city and its commercial capital, and many pharmaceutical companies keep their head offices and main distribution here. Aga Khan University Hospital, Jinnah Postgraduate Medical Centre, Civil Hospital and a dense private sector give the city some of the country's biggest prescription volumes.

Karachi territories are drawn by district and corridor — a rep might cover the Saddar–Clifton belt, the Gulshan and North Nazimabad corridors, or the industrial Korangi side. Travel time is a real factor in this city, and managers value reps who plan routes tightly and know their areas street by street.
